What this covers.
This policy describes how Maz Ghodsian, D.O. ("we," "us," "the practice") collects, uses, and protects information you provide through this website and during your care. The current version is always available at https://drmazghodsian.com/privacy/. This page is a plain-English summary; the practice's full HIPAA Notice of Privacy Practices is the authoritative document for protected health information and is available at any office.
Mobile numbers & SMS opt-in — not sold or shared for others' use.
If you opt in on our website or by another method we offer to receive text messages from this practice, your mobile opt-in (your consent and the associated mobile number used for that program) will not be shared with third parties or other companies for their own purposes. We do not sell, rent, trade, or transfer mobile numbers or SMS consent to affiliates, partners, lead brokers, advertisers, data brokers, or unrelated businesses. SMS consent is not shared with third parties.
Third parties and affiliates will not use your mobile opt-in for marketing or for sending their own messages. We use your number and consent only so our practice can send you the appointment-related messages described in our terms (including SMS terms), and to meet legal and carrier registration requirements.
To transmit SMS, we use a messaging vendor that acts solely as our service provider under contract: it delivers the messages we approve to your number and does not use your mobile number or opt-in consent for its own marketing, resale, or disclosure to other companies.
What personal information we collect.
- Contact and identity. Name, email address, phone number, and similar details you give us when you book online, request a photo consult or telehealth visit, call or email the office, or receive care.
- Health information. Clinical details, images you submit, visit history, billing-related notes, and other protected health information needed to evaluate and treat you.
- Website and device data. Limited technical information such as browser type, general location, pages viewed, and cookies or similar technologies used to operate this site and understand aggregate traffic. We do not use this data to identify individual visitors.
- SMS program. If you opt in to text messages, we collect your mobile number and a record of your consent (including that it was collected on our website).
How personal information is used.
We use personal information to schedule and deliver care, communicate with you about appointments (by phone, email, and — when you have opted in — SMS), respond to messages sent through the website, bill for services, and operate the practice. Medical information is also used to coordinate with other providers when necessary for your care.
The self pay structure of the practice means we do not submit claims to insurers — your medical information is not transmitted to insurance companies for billing purposes.
We do not sell, rent, or trade your personal information. We do not use or disclose mobile opt-in or mobile numbers for third-party marketing. SMS consent is not shared with third parties.
Who personal information is shared with.
We do not sell your personal information. Mobile opt-in and the mobile number you provide for our SMS program are not shared with other companies for those companies' own business, marketing, or resale. Other than as described in the "Mobile numbers & SMS opt-in" section above, we do not provide your SMS signup data to affiliates or unrelated third parties.
For general operations (separate from selling or sharing opt-in for others' use), we engage service providers that perform specific functions we direct — for example, secure website or email hosting, telehealth or scheduling software, payment processing where used, and our HIPAA business associates where applicable. They may process information only as needed for those services, under contract and confidentiality terms. Where they receive protected health information, HIPAA business associate agreements apply when required. These vendors do not receive your information so they can market to you or sublicense it to other companies.
We may also disclose information when required by law, to public-health or regulatory authorities as permitted by law, or to protect the safety of patients, staff, or the public.
SMS consent is not shared with third parties or affiliates for their own marketing purposes. We use consent and mobile numbers for our practice's appointment-related messaging and compliance obligations — not for sale or for other companies' campaigns.
Text messages (SMS).
When you provide your mobile number and agree on our appointment form, you consent to receive text messages from the practice at that number about scheduling and appointment-related updates. Message frequency varies. Message and data rates may apply. Reply STOP to opt out; reply HELP for help or call the office.
Your mobile opt-in will not be shared with third parties. SMS consent is not shared with third parties. We do not give your consent or mobile number to other companies to contact you for their own purposes.
For complete SMS terms and conditions (message types, frequency, rates, STOP/HELP, and related terms), see https://drmazghodsian.com/terms/.
How we protect it.
The practice maintains administrative, physical, and technical safeguards consistent with HIPAA requirements. Telehealth and photo consult submissions are transmitted through secure, encrypted channels. Only the clinical staff directly involved in your care has access to your records.
Your rights.
You have the right to inspect and copy your medical records, request amendments, request restrictions on disclosure, request communication through alternate means or locations, and receive an accounting of certain disclosures. Requests should be submitted in writing to the office.
Cookies & analytics.
This website uses cookies to keep the site running smoothly and to understand aggregate visitor behavior. You can disable cookies in your browser settings; the site will still function. We do not use third-party advertising trackers.
